Update: inhaled insulin, exenatide, and sitagliptin

Key point: New warnings have been issued for exenatide (Byetta—Amylin) and sitagliptin (Januvia—Merck), and poor sales of insulin human (rDNA origin) inhalation powder (Exubera—Pfizer)—despite years of eager anticipation—have forced this product off the market.

Finer points: On October 18, 2007, Pfizer announced plans to no longer promote or sell Exubera, stating that it will be returning the worldwide licensing rights to Nektar Therapeutics. In a Dear Healthcare Provider letter, Joseph Feczko,MD, Pfizer Chief Medical Officer, wrote that the decision to discontinue marketing Exubera was not attributable to a safety issue but because the product had not met the needs of prescribers and patients.

FDA advisory committees ruling on rosiglitazone: Risk but benefit, choice. (Full Text)

Key point: Because of analyses showing an increased risk of heart failure, FDA is requiring addition of a boxed warning to the product labeling of rosiglitazone (Avandia—GlaxoSmithKline [GSK]), pioglitazone (Actos—Takeda; Lilly), and combination products containing either of these thiazolidinediones.
