ACP Journal Club: What's New in May 2009

ACP Journal Club
by Brian Haynes, MD, PhD
American College of Physicians (2009)

ACP Journal Club
, published by the American College of Physicians, is the quintessential evidence-based journal and is often positioned near the top of the EBM pyramid. Published monthly, it selects articles from the biomedical literature that report original studies and systematic reviews that warrant immediate attention by physicians attempting to keep pace with important advances in internal medicine. These articles are then summarized in value-added abstracts and commented on by clinical experts.

May 2009 - Volume 150, Number 5

Therapeutics

Review: Pneumococcal vaccination is not effective for preventing pneumonia, bacteremia, bronchitis, or mortality Selective decontamination of the digestive tract and selective oropharyngeal decontamination reduced mortality in the ICU

Review: Evidence is insufficient to evaluate effectiveness of antiviral therapies for chronic hepatitis B Fractional flow reserve-guided PCI reduced major adverse cardiac outcomes compared with angiography-guided PCI in multivessel CAD Benazepril plus amlodipine reduced cardiovascular risk more than benazepril plus hydrochlorothiazide

Quality Improvement

Intensive primary care treatment reduced cardiovascular risk factors at 1 year in screen-detected type 2 diabetes

Therapeutics

Irbesartan did not reduce all-cause death or CV hospitalization in heart failure and preserved ejection fraction Review: Adding dexamethasone to standard therapy reduces short-term relapse for acute migraine in the emergency department

Diagnosis

Review: Computed tomography angiography is highly accurate for diagnosing significant stenosis in peripheral arterial disease

Quality Improvement

Implementation of an evidence-based feeding guideline did not reduce mortality in intensive care units

Clinical Prediction Guide

The Glasgow-Blatchford Bleeding Score identified patients with upper GI bleeding who could be managed as outpatients Absolute CVD risk, stratified by risk score, was 20% higher in primary care patients with CVD than in those without CVD
